Premier Wellness Dental Plan Membership Fees: *
$299 per year for individual coverage
$499 per year for a couple
$99 per year for a child (age 18 and younger)
Premier Wellness Dental Plan Features: *
-Two cleanings and exams a year plus necessary x-rays per patient per year.
- Pano excluded
-Save 25% on all other dental procedures* including additional x-rays, emergency exams, fillings,
root canals, crowns, and dentures. *Excludes Invisalign and Implants
-Members must use our network of dental practices.
- A reduced fee of $89 each for periodontal maintenance.
*We do offer a full refund if membership is canceled in writing within 15 days of the signed application and if no services were performed. Once services are performed with this plan, patients are not eligible for a refund.
